A 3-year-old child presented with bilateral proptosis with associated exposure keratitis and exotropia. On clinical examination there was 'frog-like' facies with flattened nasal bridge, 'parrot-beaked; nose and hypoplastic supraorbital ridge [Figure - 1] Rest of the anterior segment and posterior segment of both eyes were within normal limits. There were no syndactaly and no dental abnormalities.

Ultrasound examination showed no orbital mass, however, there was excessive fluid collection around the optic nerve, which prompted an x-ray examination of the skull [Figure - 2]a & b followed by CT head [Figure - 3], and CT orbit [Figure - 4].
