A fifty-four-year old man presented with a subtrochanteric fracture of the right femur, after having sustained a trivial fall. The x rays of the right upper femur showed a transverse fracture with sclerosis of bone [Figure - 1]. Clinically the face was small with a big nose, large head, prominences of both the frontal and parietal bones and an open fontanelle. He had a high arched furrowed palate, short stubby hands and feet and a short stature. Arm span was less than the height. No abdominal organomegaly was present. X-ray of the skull [Figure - 2] and hand [Figure - 3] were done.
