A sixty year old male, known to have Diabetes mellitus for past ten years, developed high grade fever with nausea for one week. His body temperature and pulse rate were increased while the urine output decreased. His general condition was poor with no abnormality in cardiovascular, respiratory and nervous systems. Fundoscopy showed findings of diabetic retinopathy. Abdominal examination revealed a non-tender lump in the left lumbar region. Blood urea and serum Creatinine were 141mg% and 6.3 mg% respectively. Serum sodium level was low while Potassium level was high. Erect plain X-Ray abdomen and Contrast enhanced CT(CECT) were done [Figure - 1] and [Figure - 2].
