A twenty-five-year old man complained of lethargy, weakness in both the lower limbs, difficulty in walking and backache since 20 years with recent exacerbation. No neurological deficit was found. MRI of the lumbosacral spine revealed borderline bony spinal canal stenosis and hypointense marrow changes on both T1 T2 weighted images (WI). Both iliac bones revealed more or less symmetrical lesions in the subarticular regions with sparing of the sacroiliac joints. Left sided hydronephrosis detected on MRI was followed up with ultrasound, which revealed bilateral renal calculi and left ureteric calculus causing hydronephrosis.
